For Intravenous Therapy Training:
-Request Letter for IVTT
-Photocopy of Transcript of Records
-Photocopy of Boards Rating/Certificate of Passing
-PRC Card (I don't really know what is this..hahaha.Maybe the License)
-2x2, 1x1, Passport Size photos (at least 2 for each size)
-Filling for Applications is until April 26, 2011

For Volunteer Nurse (GRBASMH):
-Application Letter
-Resume (Curriculum Vitae)
-Photocopy of Transcript of Records
-Photocopy of Board Rating/Certificate of Passing
-PRC Card
-Medical Certificate (Urinalysis, Fecalysis, Drug Test, X-ray, etc.)
-2x2, 1x1, Passport Size photos (at least 2 for each size)

-Must pass the Qualifying Exam and Interview
-IVTT Certificate
